The out-of-band KB5129195 update patches CVE-2026-62721 and partially restores USB audio, but AMD Radeon instability and File Explorer crashes reported after September's KB5124008 remain unaddressed.
Microsoft has issued an out-of-band emergency update for Windows 11, designated KB5129195, and is distributing it to everyone, with the package downloading automatically across every PC. According to the company, the release fixes crucial issues and patches privilege elevation vulnerabilities. Its changelog includes a fix for CVE-2026-62721, repairs for remote desktop connection problems, and — significantly for peripheral owners — a partial fix for the faulty USB audio behavior that has plagued machines since September.
The audio trouble traces directly to September's update, which disrupted USB audio output on many PCs, hitting USB Audio Class 1.0 devices — on systems that previously worked, sound simply vanished. Today's patch restores 8-channel and 3D audio modes, but Microsoft concedes several issues remain: some USB audio devices still lack volume controls, and others produce no audio output at all, meaning the problem is not fully resolved.
Alongside the addressed items, a batch of unconfirmed issues linked to September's KB5124008 update remains open, with outlets such as Windows Latest keeping track of the user reports. Today's out-of-band KB5129195 does not fix any of them.
Most consequential for graphics hardware users, AMD Radeon owners are reportedly experiencing system freezing, crashing, driver timeouts, black screens, and missing display output — and some encounter immediate errors even after reinstalling the driver stack. These complaints remain unconfirmed and unaddressed by KB5129195.
Separately, Windows Latest has also spotted users complaining that File Explorer fails to start or crashes immediately, while others report being unable to access their files. Neither complaint is resolved in today's emergency release, leaving affected users waiting for a follow-up fix from Microsoft.
